RESUMO

Objective:To explore the differences in clinical characteristics and treatment outcomes between patients with type A and type B alcohol dependence, and to find the independent risk factors of relapse.Methods:Alcohol-dependent male patients attending the Addiction Medicine Center of Beijing Huilongguan Hospital from January 2018 to December 2020 were selected for the study and divided into type A alcohol-dependent group ( n=77) and type B alcohol-dependent group ( n=87). All patients were given acute detoxification treatment and were followed up after treatment on relapse to drinking. Differences in demographic and clinical data were compared between the two groups, and differences in treatment outcomes between the two groups at different time points over 3 months were compared. Patients were divided into relapse group and non-relapse group according to whether they drank again after 3 months. Logistic regression model was established to screen the risk factors of relapse of alcohol-dependent patients by SPSS 25.0 software. Results:There was no significant difference between the two types of patients in years of education, marital status, smoking status and working status(all P>0.05), but the proportion of co-residents( χ2=5.69, P=0.017) and the proportion of positive family history of alcoholism were significant difference between the two type of patients( χ2=13.32, P<0.001). There were statistically significant differences between the two types of patients in the onset time( t=-7.28, P<0.001), the first drinking age( t=-2.36, P=0.020), the proportion of drinking in the morning( χ2=7.83, P=0.005), psychotic symptoms( χ2=4.31, P=0.038), convulsions after withdrawal( χ2=5.30, P=0.021), and alcohol use disorder identification test(AUDIT) score( t=4.30, P<0.001). At the 4th and 8th weekend of the follow-up, there were statistically significant differences in drinking frequency(0(0, 3), 0(0, 0), Z=-4.13, P<0.001; 3(0, 3), 0(0, 3), Z=-4.42, P<0.001) and relapse rate (40(45.98%), 9(11.69%), χ2=22.92, P<0.001; 61(70.11%), 24(31.17%), χ2=24.82, P<0.001) between the two types of alcohol dependence patients after drinking again. After 12-week follow-up, there were statistically significant differences between the two types of alcohol-dependent patients in the interval of first drinking(20(7, 30)d, 88(38, 90)d, Z=-7.83, P<0.001), the cumulative duration of abstinence(4(0, 8)weeks, 12(4, 12)weeks, Z=-5.13, P<0.001), the cumulative rate of abstinence(71(81.60%), 25(32.47%), χ2=40.62, P<0.001), the frequency of drinking after abstinence(3(3, 3), 0(0, 3), Z=-5.54, P<0.001), and the reduction of daily average alcohol consumption( t=3.36, P<0.001). Logistic regression model showed that type B alcohol dependence ( OR=3.121, P=0.03, 95% CI: 1.12-8.72) and AUDIT score ( OR=1.498, P<0.01, 95% CI: 1.29-1.74) were the risk factors for relapse of alcohol-dependent patients. Conclusions:Patients with type A and type B alcohol dependence have obvious differences in clinical characteristics and treatment outcomes, and type B alcohol dependence is independent risk factor for relapse to drinking in alcohol-dependent patients, which validate the rationality and necessity of alcohol dependence subtypes.

RESUMO

Objective:To explore the relationship between early maladaptive schemas, defense styles and mental health in patients with depression, and to investigate the mediating effects of defense styles between early maladaptive schemas and mental health. Methods:A total of 87 outpatients with depression were assessed with the Young schema questionnaire-short form 2(YSQ-SF2), defense style questionnaire(DSQ), symptom checklist-90(SCL-90) and Beck depression inventory(BDI). SPSS Process program and Bootstrap method were used to explore the mediating effect of defense styles. Results:The scores of abandonment/instability were higher in females than those in males (2(0.5, 4), 0(0, 1.25), Z=-2.963, P=0.003) while the score of emotional inhibition was higher in males than in females (1(0, 3), 0(0, 2), Z=-2.318, P=0.02). The total score of SCL-90 was positively predicted by vulnerability to harm or illness and emotional deprivation, with an explanatory variation rate of 27.7%.The number of SCL-90 positive items was positively predicted by social isolation/alienation and abandonment/instability, with an explanatory variation rate of 19.3%.Immature defense styles fully mediate the relationship between social isolation/alienation and mental health, as well as between abandonment/instability and mental health. Conclusion:Social isolation/alienation and abandonment/instability affect the mental health of patients with depression by the mediating effect of immature defense styles.

RESUMO

Objective:To investigate whether ginkgo biloba extract(EGb) can improve the symptoms of tardive dyskinesia(TD) by increasing the levels of serum brain-derived neurotrophic factor (BDNF), and to explore whether the BDNF Val66Met genotype can influence the efficacy of TD by EGb.Methods:A total of 78 male schizophrenia inpatients with TD were enrolled, and 77 patients completed 12 weeks of treatment with EGb(240 mg/d). The abnormal involuntary movement scale (AIMS), positive and negative syndrome scale (PANSS) and serum BDNF levels were measured at before and after 12 weeks of treatment.Serum BDNF levels were measured by enzyme linked immunosorbent assay (Elisa). In addition, the BDNF Val66Met polymorphism was genotyped by the method of PCR-restriction fragment length polymorphism (PCR-RFLP) in all patients. Results:Compared with before treatment, the scores of total AIMS((7.0±2.9) vs (4.9±2.2)), total PANSS ((55.8±14.0) vs(51.1±9.7)), subscale P((9.6±3.3) vs (8.6±2.2)), subscale N((23.2±8.3) vs (21.4±6.3)) and subscale G((23.0±4.9) vs (21.1±2.7)) were significantly decreased( P<0.01), while the levels of BDNF were significantly increased((10.8±2.9)μg/L vs (9.6±3.3)μg/L, P<0.05) in TD patients after treatment. The increased level of BDNF was positively correlated with the decrease of AIMS( r=0.28, P=0.014), but no significantly correlated with the improvement of PANSS total scores and the three subscales were found(all P>0.05). After treatment, the decrease of AIMS in patients with Val/Val genotype was significantly greater than that in patients with Val allele (Met/Val plus Met/Met genotype) (both P<0.05). Further analysis showed that only patients with Val/Met heterozygotes had a significant increase in BDNF levels after treatment ( P<0.05). Conclusions:EGb may improve TD symptoms through neuroprotective and regulatory effects. The BDNF genotype is involved in regulating the efficacy of EGb in the treatment of TD, which may be related to its regulation of BDNF serum levels.

RESUMO

Objective To investigate the level of serum vascular endothelial growth factor (VEGF) in stable schizophrenia patients, and to explore the relationship between serum VEGF level and clinical symptoms. Methods Seventy-five patients with stable schizophrenia and 40 healthy controls were enrolled. The levels of serum VEGF were detected by flow multiplex protein analysis. The clinical symptoms of patients were assessed by positive and negative symptom scale (PANSS). Results The level of serum VEGF was significantly lower in patients group than in healthy control group [medians (lower and upper quartiles) were 3.77 (2.61, 5.14) pg/mL vs. 6.21 (4.37, 11.16) pg/mL, P<0.01]. There was no significant difference in serum VEGF levels between patients with different gender, smoking or family history of mental illness (P>0.05). The level of VEGF was negatively correlated with the total score of PANSS (r=-0.27, P=0.03), negative symptom subscale score (r=-0.25, P=0.04), lack of response score (r=-0.26, P=0.02), but not with other PANSS scores (P>0.05). Conclusion The serum level of VEGF in patients with stable schizophrenia is lower than that in healthy control group. There is, to some degree, a relationship between the level of VEGF and the severity of clinical symptoms in patients with stable schizophrenia.

RESUMO

This paper briefly introduces the structure and function of α7nAChR, and mainly reviews the relevant researches about the relationship between a7nAChR and cognition of schizophrenia in the last ten years from molecule genetics and new targets of medication two aspects. It suggests that the abnormal α7AChR is involved in cognition impairment of schizophrenia, and the a7nAChR agonist is probably able to improve the impaired cognition in patients with schizophrenia. Furthermore, KAT-2 as a key enzyme acting on its endogenous antagonist KYNA is also promising to become a new drug target

Objective:To investigate the roles of S100B protein and anti-brain antibody (ABAb) in the pathophysiology of Alzheimer's disease (AD) by analyzing the changes of the serum levels of S100B and ABAb and the relationships of the measures with cognition deficits in patients with AD.Methods:In this study,32 patients with AD(AD group) and 40 age-matched volunteers without cognitive impairment(control group) were enrolled. The diagnosis was made according to the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition (DSM-Ⅳ).The mental and social functional conditions were assessed with the Mini-Mental State Examination (MMSE) and Activity of Daily Living Scale(ADL),the cognitive function of patients was evaluated with the Alzheimer's Disease Assessment Scale-cognitive subscale(ADAS-Cog).The serum S100B proteinand ABAb levels were examined by enzyme-linked immuno sorbent assay(ELISA).Results:The serum S100B protein[(0.66 ± 0.17) μg/L vs.(0.30 ± 0.04)μg/L] and ABAb [(1.93 ± 0.95) U/L vs.(1.31 ± 0.25) U/L] levels were higher in AD patients than in the controls (Ps < 0.01).In AD patients, the serum S 100B protein markedly negatively correlated with the scores of the MMSE(r =-0.66),while positively correlated with ADL and ADAS-Cog(r =0.57,r =0.53)(Ps < 0.005).ABAb levels negatively correlated with the scores of the MMSE(r =-0.57),while positively correlated with ADL and ADAS-Cog(r =0.52,r =0.34)(Ps <0.05).The serum S100B protein levels were positively related to ABAb levels in AD group(r =0.51.P <0.005),but not in control group(r =0.076,P =0.654).Conclusions:It suggests that the serum levels of S100B protein and ABAb are related with cognitive function in patients with Alzheimer's disease, and S100B protein and ABAb might play key roles in mechanism of Alzheimer's disease.

RESUMO

Objective To investigate the efficacy of treatment and prevention of VitE on vacuous chewing move-ments (VCMs) of haloperidol-induced tardive dyskinesia (TD) rats and serum levels of brain-derived neurotrophic fac-tor ( BDNF) and total antioxidant capacity ( TAC) , and to explore the possible mechanisms. Methods Thirty-two male Sprague-Dawley (SD) rats were randomly divided into TD, P-Vit E, T-Vit E and control group (n=8), receiving to-week treatment with Haloperidol (Hal)+NS, Hal+Vit E (medicated at the baseline), Hal+VitE (medicated at the fifth week) or normal saline (NS), respectively.VCM was evaluated at each week.ELISA and spectrophotometer were used to detect the serum levels of BDNF and TAC, respectively. Results The VCM score of both TD group and T-Vit E group increased at the 2nd weekend, reached the peak at the 5th weekend.VCM score of T-Vit E group declined gradually at the 6th weekend and was significantly lower than that in the TD group [(6.5 ±3.3) vs.(27.9 ±5.8), P0.05) at the 10th weekend. There was no significant difference in VCM score between P-Vit E group and control group for ten weeks(P>0.05).At the 10th weekend, serum BDNF [(6.9 ±1.0) pg/mL] and TAC [(11.9 ±3.2) U/mL] levels of TD group were significantly lower than those of the controls [BDNF (8.6 ±2.5) pg/mL, TAC (18.2 ±5.5) U/mL] and T-Vit E group [BDNF (8.7 ±2.0) pg/mL, (18.6 ±5.9) U/mL] (P0.05).Conclusions Vit E may relieve and prevent VCM in TD model rats though alleviation of free radical damage.

RESUMO

Objective:To explore the causes of the maintenance of smoking behaviors in male patient with chronic schizophrenia. Methods:Totally 188 male, smoking patients with chronic schizophrenia, meeting with the criteria of Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition (DSM-IV),above 5-year course of disease, taken a steady dosage of antipsychotics for 3 months, and 61 healthy smoking men were collected according to the voluntary principles. The psychopathologic symptoms and the dependence of cigarette of all patients were as- sessed with the Positive and Negative Syndrome Scale (PANSS)and the Fagerstrom Test for Nicotine Dependence (FTND),the Russell's Smoking Motivation Questionnaire (SMQ)was used to investigate the causes for maintai-ning behaviors of smoking in patients and healthy controls respectively. Results:The sedative item scores of SMQ were lower in patient group than in the controls [3 (0,9)vs.6 (0,9),P <0.001],and other scores of subscales had no significant difference compared with control.Heavy dependence happened in patient group slightly less than health controls (2.7%vs.8.2%,P <0.05).The total scores of FTND were positively correlated with the doses of antipsychotics (chlorpromazine equivalent)(r =0.22,P <0.01).The scores of total (β=0.22),automatic (β=0.27)and sedative (β=0.37)items in patient group were less than controls (Ps <0.001)by multi-factors step-wise Linear regression analysis. Conclusion:Compared with general smokers, lower need for sedation, less heavy dependence and motivation in smoking patients might be correlated with the strict management for inpatients smok-ing,social-psychological factors and culture background in China.

RESUMO

Objective To explore the reliability and validity of the computerized Chinese version of Cambridge Prospective Memory Test (C-CAMPROMPT) for assessment of prospective memory (PM) in chronic schizophrenia patients. Methods 50 patients and 50 healthy controls formed the study sample. PM performance was measured with computerized C-CAMPROMPT, while the Wechsler Adult Memory Scale-Forth Edition (WMS-IV), Wisconsin Card Sorting Test (WCST) and Category Fluency Test (CFT) were administered to assess logical memory (LM), visual representation (VR), executive function and processing speed. Results The test-retest reliability (0.981, P<0.001), split half reliability (0.627, P<0.001) and internal consistency reliability (0.742) of C-CAMPROMPT were satisfied. The scores of C-CAM-PROMPT and its subtest in schizophrenia were lower than that in healthy control (P<0.001). The performance of PM in patients with schizo-phrenia closely related to the scores of LM, VR, WCST-CC and CFT (r=0.34~0.89, P<0.05). The sensitivity (86%) and specificity (92%) of the scale were satisfied. Factor analysis extracted 2 factors. Conclusion The computerized C-CAMPROMPT shows a good reliability and validity for assessment of PM function in chronic schizophrenia, and is a sensitive, adaptable, stable instrument.
